Sonoma ACT (Assertive Community Treatment) is a comprehensive, community-based program for individuals with serious mental illness (SMI). Services are provided at the locations where problems occur such as clients homes, transitional housing vs. hospital or clinic settings. The program serves a total of 50 Clients.

POSITION OBJECTIVE

The Personal Service Coordinator II provides direct client community support and case management services to assigned members using Psychiatric Rehabilitation and Recovery principles. Provides education and support to members and their families. Collaborates with other services and agencies such as the Conservator's Office, Public Defender's Office, Child Protective Services, Probation Department and County Mental Health Services. Completes assessments and determines the appropriate level of services for new members.

QUALIFICATIONS

-High School Graduation or G.E.D. equivalent and four years of direct services in Mental Health; or Associate Arts degree and two years of direct services in Mental Health; or BS degree in Social Sciences and one year experience in Mental Health Services are required.

-Experience will have been in a mental health setting, outpatient clinic, or community mental health service preferred.

-Experience in providing intensive case management, intensive community supports using psychiatric rehabilitation concepts is highly desirable.

-Sensitivity to multi-cultural populations and issues is required. Necessary skills include: the ability to work effectively as a member of a multidisciplinary team, a thorough understanding of client rights, an understanding the conservatorship process, and knowledge of basic medical and psychiatric terminology.

-Must have a valid California drivers license, a DMV clearance for insurability, a vehicle (approved for safety), and proof of insurance.

-Applicant must be fingerprinted and receive clearance from the Department of Justice and The Office of inspector General.

-Must possess a valid drivers license, proof of insurance, and receive a motor vehicle record clearance.

-Must have completed first aid and CPR training or obtain certification within four (4) weeks of hire.
